REQUESTING A BIRTH, DEATH, OR MARRIAGE CERTIFICATE

Birth Certificates

Births are recorded in the town where the child was actually born, with a certified copy being forwarded to the town where the parents reside.  We will not have certificates unless one of those two conditions is met.

Access to certified copies of birth records less than 100 years old is restricted in Connecticut.  Persons 18 yrs of age or older permitted access include the person whose birth was recorded, the person’s children, grandchildren, spouse, parent, guardian or grandparent, attorney representing the registrant or registrant’s parent, child, guardian or surviving spouse, head of the municipality and local director of health where birth occurred, and certain genealogical societies registered in the State of Connecticut.

A certified copy of a birth certificate of an adopted person may be issued; however, the original birth record remains sealed unless ordered open by the Court.

Certified copies of birth certificates may be obtained for $20.00.

Issuance of a birth certificate is regulated.  The request must be accompanied by a government photo ID of the person making the request, or 2 other forms of approved identification.

Verification of the relationship to the person named on the certificate must be provided; for example:

    • If you are requesting a copy of your grandchild’s birth certificate, you must also submit a copy of your child’s long form birth certificate (unless said parent was born in Windsor Locks – then just give us the date of birth).
    • If you are requesting a copy of your spouse’s birth certificate, you must also submit a copy of your marriage certificate (unless the marriage took place in Windsor Locks – then just give us the date of the marriage).
    • If you are requesting a copy of your parent’s birth certificate, you must also submit a copy of your long-form birth certificate (unless you were born in Windsor Locks).

Death Certificates

For deaths that occur in the Town of Windsor Locks, completed Death certificates are filed by funeral directors and embalmers with the Registrar of Vitals in the Town Clerk Office.

Anyone, over the age of eighteen years, may request to purchase a certified copy of a specific death record.  The fee for a certified copy is $20.00.

Unless the death certificate is being issued to a surviving spouse or next of kin (or a funeral director within 60 days of disposition), social security numbers will be redacted for all deaths occurring after July 1, 1997.   Proof of relationship to the deceased will be required before a certified copy with social security numbers is issued.

We also issue Burial, Cremation and Disinterment permits.

Marriage Certificates

Certified copies of your marriage license can be obtained from the Clerk’s Office where the marriage occurred or your Town/City of residence at the time of marriage.
